Project BRAVO administers the Patient Assistance Program (PAP) to address the need for health care services. PAP helps patients obtain, complete and submit applications to the manufacture companies that provide free and reduced medications, diabetic supplies, smoking cessation inhalers, and nutritional supplement drinks.
The PAP Program also provides assistance in completing and submitting applications for a free eye exam.
The PAP Program has established a partnership with an optical that provides free eyewear to patients that are financially unable to obtain their eyeglasses.
Not only does the PAP Program assist the patients with free medications, eye exams and eyeglasses but also identifies and provides referrals for free or low cost health clinics, community health centers, hospital benefit plans and local pharmacy discount programs.
This program meets the needs of some 400 patients each year.
Documents needed at time of appointment for medications:
Please bring to your appointment the following documentation:
• Picture I.D. of everyone in household over 18
• Social Security Card for everyone in the household
• Proof of Income for the last 30 days
• Property tax/rent or mortgage payment receipt
• If you receive Medicare or Medicaid, bring your card or form
• Names of the medications (preferably a prescription or the bottles were the medication was filled previously)
• Name, address, zip code, and telephone number for doctor prescribing the medication
• There is a fee of $10.00 for the application process (fee can be waived on a case by case basis)
