JOB DESCRIPTION

Job Title:  Caseworker II

FLSA Exemption Status:    Non-Exempt                                    Department: Community Services

 

PRIMARY FUNCTION:   Caseworker II’s are responsible for providing social services to clients and their families by assisting them with personal and environmental difficulties. Caseworker II’s work closely with other agencies to further their understanding of significant social and emotional factors underlying client's needs. They also serve as a resource referral service in advising clients and their families about available community resources.

DUTIES: 

  • Perform financial screening for intake assessments to determine client's eligibility for assistance from TDHCA funded programs. 
  • Identify individuals or families that will be assisted with the necessary supportive services to become self-sufficient.
  • Develop a self-sufficient plan with input from client and family and coordinate overall case management for the client in support of Project Bravo’s mission.  
  • Make client referrals to community sources.  Cooperate with social service agencies to develop and maintain a coordinated social service program. 
  • Develop strategies to recruit and engage community and grassroots leaders to form Neighborhood Improvement Associations (NIAs).
  • Provide technical assistance to NIAs in the planning of initiatives to improve their community such as neighborhood watch groups, clean-ups, debating issues on rezoning of their neighborhoods, etc.
  • Assist NIAs with the development of by-laws, strategic planning, leaderships, and board training. Form NIA groups, and presentations, perform outreach. 
  • Maintain accurate records documenting all activities and submit timely reports to supervisor. 
  • Ability to interact and communicate with staff, clients, volunteers, agency representatives, and other non-profit organizations in the community.  
  • Maintain an effective relationship with staff, agencies, and grassroots groups to maintain high visibility and an outstanding public image. 
  • Ability to adjust to changes in the operations of the agency, learn new skills and make effective decisions which reflect sound knowledge and good judgment. 
  • Perform other duties as assigned.

 

DELIVERABLES: 

  • Complete reports such as case management, DIS forms, and elderly credits.
  • Conduct outreach on a monthly basis as needed.
  • Maintain 3 case management and provide follow up.
  • Serve customers, refer them to needed community resources,  ensure that required documentation is inserted and secured in file, and enter information accurately and completely in SHAH.
  • Accurately determine eligibility.
  • Adhere to policy and procedures.
  • Register NPI’s accurately to reflect service provided.
  • Acquire signature of customer and staff member on all documents.
  • Assist in the development and maintenance of NIA

(Neighborhood Initiative Association).

 

PHYSICAL:

Sit, stand, stoop, talk, and walk, reading and writing.  Ability to lift 20-25lbs. 

 

SKILLS/KNOWLEDGE:  Knowledge of social service components, with experience in non-profit organizations,   community outreach, intake, and assessment.  One year experience in Case Management.  Proficient in Word, Excel, Microsoft Outlook, and other database driven computer technology.    

 

MINIMUM QUALIFICATIONS:  Associate's Degree in Social Work from an accredited program OR one (1) year of Social Work courses and two (2) years of directly related experience.   Education will substitute for two years of experience.  Must have excellent interpersonal and communication skills.  Ability to work evenings, weekends, and holidays.   Must be bilingual in English and Spanish.  Must possess a valid driver's license and liability car insurance.  Must be able to utilize a personal vehicle for company business.    

 

REPORTING STRUCTURE: This position reports to the Community Center Supervisor.
